The story takes place in 1945, during the Japanese occupation of Korea. The military scientist created a worm that attacked the brains of the people and made them mutant.

Tae-sang is a pawn shop owner who has a large network of informers who collect details about missing persons. Yoon Chae-ok and her father came to the town in search of her mother, Sheishen, who had been missing for years. They seek help from Tae-sang to find her mother, and their investigation leads to the Onseong Hospital, which discovers that many people are trapped in the basement.

Due to the experiment on her, Tae-sang’s mother turned into a creature, but she still recognized her daughter. Sheishen does not attack her daughter; instead, she protects Tae-sang from the Japanese soldiers.

In the final episode, it is revealed that Lady Maeda is the antagonist, having given funds for the experiment. She brings Chae-ok to the scientist to make Chae-ok a monster. But a dynamite explodes, destroying the lab. Her mother helps her fight back the soldiers, but one of Sheishen’s tentacles stabs Chae-ok, and after that, she seemingly dies. But then she opens her eyes as they show the worms working on her body.

The first season ended with many unanswered questions. Myeong-ja accidentally drinks the water mixed with Nijan, which leads her to become a creature. She then gives birth to a half-human/half-creature child. Lady Maeda escapes from the explosion but gets burned and is in a wheelchair.
