The series is set at the start of the 17th century in a fictional Joseon Dynasty and follows Lee Chang, the Crown Prince of Joseon. His father and the King of Joseon became ill from deadly smallpox for ten days and were only allowed to see his pregnant wife, Queen Consort Cho, and her father, Haewon Cho clan.

Lee Chang was not allowed to see his father by his stepmother, Consort Cho. He is aware that his life was in danger once his baby brother was born. He decided to investigate the mysterious illness that caught the king with the help of his bodyguard, Mu-yeong. The investigation led him to be caught in the middle of a deadly epidemic that was spreading rapidly in the kingdom of Joseon. He must save the kingdom from the epidemic and also stop the people who are trying to seize the throne.

The third season will continue where the second season concluded and answer why the zombie outbreak happened. Lee Chang learned that a mysterious woman named Ashin was spreading the virus and keeping zombies alive. Season 3 will explore her intentions and where she came from.
